What to bring on your 1st visit:
- Please arrive 15 minutes early to fill out forms.
- Insurance Cards
- Doctor’s prescription
- List of medications
- Please wear loose fitted clothing. For lower body injuries: hip to feet it is recommended to wear shorts but is not required.
You may also bring clothes to change into.
What to expect on your 1st visit:
- Evaluation with therapist
- Development of personal treatment plan based on medical necessity.
- Partial treatment may be included (Determined by insurance)

Financial Policy
As a courtesy to our patients Phelps & Associates, Physical Therapy will check your insurance coverage. However, we cannot guarantee benefits will be paid as quoted by your insurance. Patient’s portion is expected at the time services are rendered. We suggest you contact your insurance company to check your plan benefits for Physical Therapy. Please check with the front office if you have any questions. It is your responsibility to understand your specific insurance plan and its coverage. It is our policy to bill your insurance carrier as a courtesy to you, although you are responsible for all charges incurred. We do accept Cash, Check, Visa, or MasterCard as method of payment.
Insurance
Because of our long history of treating a large population of patients we are often asked by the insurance companies to be a “preferred provider”. A “preferred provider” is a healthcare provider that has a contract with the insurance company to accept the “contracted rate” of billed charges agreed by the insurance company and the healthcare provider. By accessing a “preferred provider” through the insurance company it can greatly reduce “the patients” out-of-pocket expense.
Due to economical times we have agreed to help our patients with their out of pocket expense by agreeing to be “preferred providers” for several insurance companies. This helps our patients by reducing their out of pocket expense (deductibles & co-insurances/co-pays) but at the same time delivering the same quality care.
